Important Privacy Choice

You have the right to restrict the sharing of your name, address, and electronic mail address with our affinity partners. The following opt-out options do not prohibit us from sharing your information when we are required to do so by law. This includes sending you information about the Alumni Association, university, or other products or services.

Restrict Information Sharing with Affinity Partners

Unless you say "NO", we may share your name, address, and electronic mail address with our affinity partners. Our affinity partners may send you offers to purchase various products or services that we have agreed they can offer in partnership with us.

Time Sensitive Reply

You may decide at any time that you do not want us to share your information with our affinity partners. Your choice will remain unless you state otherwise. However, if we do not hear from you, we may share your name, address, and electronic mail address with our affinity partners.

If you decide that you do not want to receive information from our partners, you may do ONE of the following:

1.	Reply electronically by using completing the form below and clicking on the "Submit" button.
2.	View a copy [PDF] of this form and then print it. Fill out, sign and send back this form to us at the following address: CSUSB Alumni Association, 5500 University Parkway, San Bernardino, CA 92407. You may also fax the form to (909) 537-7052.
3.	Email SB569@csusb.edu with the following information: first and last name, address, city, state, zip, phone number, and the following statement: "Please do not share my name, address, and email address with alumni affinity partners."
4.	Call 888-537-3093 and follow the opt-out instructions.
